Grants are Available!
For Any Man/Women Who Needs Funding:
Apply for a grant at the United Breast Cancer Foundation at least 5 business days before imaging. (The Foundation requires up to 15 days, but 8 out of 10 apps are completed within 3 business days.) Use your award (by being imaged) within 60 days. Most clients are granted between $100 and $250. NOTE: If UBCF is closed to applicants, their funds are low and they are collecting donations from corporate America. Check back every-other business day until they are accepting applications again.
For Any Breast Cancer Patient Who Needs Funding:
Apply for funding in the first 3 years of treatment/remission.
Insurance Coverage
Our service codes are covered by insurers, but we are out-of-network. Health Sharing, Flexible Spending Accounts and Health Savings Accounts typically cover 100%.
In a Sharing Program? We understand that if claimed as your Annual Woman’s Wellness Visit, Liberty HealthShare covers $400.
To learn about your out-of-network benefits*, consider posing these questions to your health insurer:
- Do I have out-of-network benefits (or only in-network benefits)?
- If yes:
- Do these benefits cover preventive services, ie CPT code 99386 (preventive medical evaluation)? Do not use the term thermography!
- What percent of such service charges are typically applied towards the out-of-network deductible (or reimbursed to me once the deductible is met)?
- What is my out-of-network deductible and have I met it this annum?
- Am I required to obtain a doctor’s prescription to obtain out-of-network coverage?
- If no, please consider using Health Savings Account funds, if you have them.
- If yes:
*Medicare does not cover non-Medicare providers. Supplemental plans may provide coverage. For instance, some Physician’s Mutual plans cover 100% of preventive services outside of Medicare providers.
We cannot guarantee coverage. While we are a fee-for-service provider, we will provide you with a helpful Superbill for use in filing a claim for reimbursement.
Pricing
While imaging is popular among men and women, imaging/reporting/self-care education with videos are priced in the $100s, dependent on geographical location.
Pricing includes more than just a report.
